  • Tobacco Core Recommendations Description Sheet (pdf) - Developed by the Taking on Tobacco in Michigan Steering Committee and approved by the Michigan Association of Health Plans' Medical Directors, these core recommendations (measures) are based on the 2000 Preventive Health Service Clinical Practice Guideline. Core measures are basic aspects of care that all patients should expect to receive from their health care provider.

  • "You Can Quit Smoking" Reference Sheet (pdf) - A personalized quit plan for clinicians to distribute to patients who are ready to make a quit attempt. The document is from the US Department of Health and Human Services/ Public Health Service.

  • Vital Signs Chart Stickers (pdf) - Labels to help remind physicians to include tobacco status as a vital sign and document the results in patients' charts. Print them in your office using an Avery 5163 label sheet.

  • Tobacco Cessation Benefits Grid (pdf), in HTML format - This document is intended to be a quick reference tool for physicians and their staff in assisting patients who are motivated to quit using tobacco. It provides information about 23 Michigan health plan's benefits regarding tobacco cessation, including programs and pharmacological aids. This grid is updated quarterly.

  • MQIC Guideline (pdf) - The Michigan Quality Improvement Consortium's Guideline for Tobacco Control. This guideline, adopted by most Michigan health plans, recommends specific interventions for cessation services for current smokers and tobacco users.

  • Pharmacotherapies List (pdf) - A table of first line pharmacotherapies approved by the FDA for tobacco cessation and suggestions for clinical use.
